[CANMEDLIB] Library Policies on lost/stolen books

Hello everyone,

Does anyone have an official policy in their library for lost and/or stolen books? I am looking to draft one so the responsibility on the patron to replace the book if not returned. (Many of my high-demand stuff goes missing despite my many attempts to contact them…)

Does anyone have any policies in their library they can share with me? Bonus points if it’s from a hospital library but I accept all kinds. ☺
